The ASCO Cancer Treatment Plan and Summary is a brief record of your cancer treatment. The forms provide a convenient way to store information about your cancer, cancer treatment, and follow-up care. The forms are also meant to give basic information about your cancer treatment and follow-up care to future health-care providers.
Information your oncologist can enter on the treatment summary includes the dose of chemotherapy you received, the specific drugs that were used, the number of cycles that were completed, and any additional treatment that was given (such as radiation therapy or hormone therapy).
The Breast Cancer Survivorship Care Plan and Colon Cancer Survivorship Care Plan were developed to help doctors and their patients develop a customized follow-up care plan once treatment is nearing completion. The recommendations for follow-up tests are based on the best available scientific evidence from ASCO's clinical practice guidelines, which are summarized in the ASCO Patient Guides.
